Are u using the ldap plugin? We hade simular problem like this and it was related to the lookup in the AD.

user-styles.css i a nice thing will try this att next upgrade, so we don't have to redo all the css every time we upgrade 🙂

That sound lika a permissions issue, the web server needs to be able to write to the themes folder. On IIS you can change the settings in the IIS MMC...
